At North Lake Care Center, we are dedicated to setting standards in home health care, and we are seeking a compassionate and skilled Per Diem LPN to join our team. As a key member of our care center, you will play a vital role in providing high-quality care to our residents.
About the Role:
As a Per Diem LPN, you will work variable shifts, including days, evenings, and weekends, to support our hospital's needs.
You will provide direct patient care, including administering medications, performing treatments, and providing wound care.
You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
You will maintain accurate and detailed records of patient care and medications administered.
Responsibilities:
• Assess and document patient condition, including vital signs, and report changes to the RN or physician.
• Administer oral, topical, and injectable medications as ordered by the physician.
• Perform basic nursing skills, such as taking blood pressures, injecting medications, and applying dressings.
• Provide wound care, including changing dressings and applying topical treatments.
• Develop and implement individualized care plans in collaboration with the RN and other healthcare professionals.
• Maintain accurate and detailed records of patient care and medications administered.
