About the Role:We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team! As a Licensed Practical Nurse, you will play a vital role in providing exceptional, person-centered care to our residents and patients.
Responsibilities:- Provide direct patient care to residents and patients, assisting with activities of daily living under the direction of a professional nurse.
- Assist in the implementation of individualized treatment plans for each assigned resident.
- Supervise CNAs and participate in their evaluation.
- Oversee Falls Program and Pressure Sore Program.
- Provide nursing services and conduct thorough evaluations of each resident’s medical status upon admission and throughout their course of treatment.
- A current, unencumbered, active state license to practice as an LPN.
- Experience in Skilled Nursing/Rehabilitation facilities preferred.
- Ability to work a flexible schedule, including evenings, weekends, and holidays.
