Sage Group Health Care  Staffing is seeking RN Hospice for a 13-week travel / contract position in West Plains, MO. Accepting both local and traveler candidates.

Pay Rate:

Range $2120 to $2200 Weekly Gross pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Unit: Home Health Hospice

Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri

Position Requirements:

• Registered Nurse: 1 year
  - Required
• Hospice Nursing experience in a home health setting: 1 year within the last 3 years – Required
• Home Care Home Base(HCHB) charting
  - Highly Preferred
• OASIS experience
  - Preferred
• Must have valid
  
  Driver’s License and Personal Auto Insurance

Education, License and Certification Requirements:

• ASN or an equivalent– Required
• BSN or an equivalent– Highly preferred
• RN License for State applying or Compact if accepted in the state
  - Required
• BLS Certification from AHA or American Red Cross
  - Required

Job Description:

• Provide and coordinate care for assigned patients in the comfort of their homes.
• Manage assigned patient case load
• Develop and implementnursing care plans, including Start of Care and admissions, as assigned.
• Support patient comfort and dignity for terminally ill patients
• Empower patients and families to reach healthcare goals by educating them on disease management.
• Provide Hospice nursing care and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
• Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
• Ensure patients’status is assessed on an ongoing basis and pertinent information is gathered and documented.
• Notify physicians of significant changes in status.
• Document and complete charting for all patient visits within 24 hours.
• Provide bereavement support to patient’s families as needed.

  West Plains, MO and possibly assisting in Mountain Grove, MO
