Franciscan Healthcare is seeking a Health Coach RN to join our team in West Point, NE. The role focuses on nursing care, case management, care transitions, diabetes and chronic disease education, and population health initiatives to empower patients in making informed health decisions.
The RN will coordinate assessment, care planning, and evaluation across the lifespan, emphasizing patient advocacy and collaboration with health professionals.
Hours:
35+ per week, Mon–Fri with rotating Saturdays.
