Job Description
The role involves conducting comprehensive medical histories and physical examinations for new participants, along with event-based and periodic reassessments.
The physician will develop and implement individualized medical treatment plans and deliver essential medical services across various residential settings for participants.
Key responsibilities include interviewing and evaluating applicants to the Programs of All-Inclusive Care for the Elderly (PACE), as well as engaging with their caregivers to contribute to the Intake and Assessment decision-making process.
The physician will discuss participants' medical conditions and prognoses with them and their caregivers, thereby facilitating informed decision-making regarding future care options.
The physician is expected to execute duties in a collaborative, multidisciplinary environment within TH PACE, contributing to the nursing treatment program for participants. This role also involves being part of the leadership team, developing plans to achieve operational objectives, and actively participating in the PACE Organization's interdisciplinary team. The physician will provide insight into diagnoses, cognitive and physical functionality, and available treatment modalities.
Participation in team and family conferences is essential, along with educating participants and their families about health maintenance and the management of chronic medical conditions, in collaboration with nursing staff. The physician will supervise the interdisciplinary team's health care and treatment programs, perform thorough physical examinations, order and interpret relevant laboratory and diagnostic tests, and systematically document results.
In addition, the physician will be required to participate in an on-call rotation as necessary.
Qualifications- Possession of a valid and active plenary physician license to practice medicine in Massachusetts is REQUIRED
. - Board Certification in Internal Medicine or Family Practice is MANDATORY

Board Certification in Geriatrics is PREFERRED
. - Experience or additional qualifications in geriatrics is PREFERRED
. - Current certification in Controlled Substances (CDS) and DEA Registration is REQUIRED
. - Previous experience in a supervisory role is REQUIRED
. - A minimum of one (1) year of documented experience with the frail or elderly population is REQUIRED
. - Demonstrated proficiency in physical assessment and chronic disease management tailored to frail older adults is REQUIRED
. - An enthusiasm for working in an innovative alternative health care setting is REQUIRED
. - Ability to manage shifting priorities in response to participant needs is REQUIRED
