Make a meaningful impact every day as a Center Well Home Health nurse. You’ll provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working closely with a dedicated team of physicians and clinicians, you’ll develop and manage care plans that support recovery and help patients get back to the life they love.
Responsibilities- Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient, providing necessary follow‑up as directed by the Clinical Manager.
- Confer with physicians to develop the initial plan of treatment based on orders and assessments; provide hands‑on care, evaluate the plan, and teach the patient in accordance with physician orders under Clinical Manager supervision.
- Coordinate appropriate care across various healthcare personnel (Physical Therapists, Occupational Therapists, Home Health Aides, and external providers).
- Report patient care, condition, and progress to the patient’s physician and Clinical Manager continuously.
- Implement patient care plan in conjunction with patient and family to achieve optimal resolution of needs and problems.
- Coordinate, oversee, and supervise Home Health Aides and Personal Care Workers, provide written instructions, and monitor documentation completion.
- Discharge patients after consultation with the physician and Clinical Manager, preparing and completing required clinical documentation.
- Prepare appropriate medical documentation for all patients, including case conferences, medication order changes, re‑certifications, progress updates, and care plan changes. Prepare visit and shift reports, summarize patient records, and confer with other health care disciplines to provide optimum patient care.
- Diploma, Associate, or Bachelor Degree in Nursing.
- Minimum of one year of nursing experience (preferred).
- Strong experience in Med Surg, ICU, ER, or acute settings.
- Home Health experience is a plus.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
Base Pay Range
$84,600 – $116,300 per year
Pay per Visit/Unit
$54.00 – $75.00
