Center for Human Development, (CHD), Children and Families Programs is seeking an LPN, who is a compassionate, patient and committed individual to work in our TLC Medical Foster Care Program. Our program is unique and employs Foster Parents and Home Health Aide and/or Medical Assistants to care for children that are medically fragile. A caseworker is assigned to the 8-10 children that we serve.
We are contracted with the Department of Children and Families (DCF) and are licensed by EEC. This is an ON-SITE position that requires travel for home visits and program needs.
Your role as an LPN with CHD:
The LPN will have oversight of the staff, clients, referrals and discharges. Because our children are often in a medical facility prior to being referred the LPN will interface with the medical community in Western and Central Massachusetts. This individual will visit the foster homes to oversee the care of the children and will attend as needed medical appointments. When referrals are made to the program, it is the responsibility of the LPN to work closely with DCF and Special Kids Special Care services.
Treatment team meetings and other collateral meetings will be attended by the LPN.
Minimum Qualifications:
- LPN in good standing.
- Previous experience working with the pediatric population preferred.
- Valid driver’s license and registered vehicle required. Use of personal vehicle for travel to participant’s foster home visits, doctor’s offices. Boston Children’s hospital and for accompanying participants for family visits. (mileage reimbursed for travel).
- Participate in on-call rotation (stipend provided).

- Strong written skills and experience with Electronic Health Record Management is a plus.
