Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Brookhaven Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team, providing exceptional end‑of‑life care throughout the community.

Schedule: Per Diem On Call Availability

Territory: West Springfield

Responsibilities:

• Empower Through Engagement:
  Respond to clinical referral information with care and expertise.
• Collaborative Care Champion:
  Partner closely with the Clinical Director to assess eligibility and suitability of clients.
• Nurturing Direct Care:
  Provide comfort and support to patients and families, ensuring quality of life.
• Nursing with Heart:
  Deliver top‑notch care rooted in proven principles and techniques.
• Educator and Advocate:
  Empower patients and families through education, encouraging active participation in care plans.
• Call of Compassion:
  Participate in call duties related to patient care.

Qualifications:

• Current unencumbered Registered Nurse license.
• Minimum one year experience as an RN in a medical‑surgical/acute care setting; hospice experience a plus.
• Computer proficiency in typing, various programs, and background in EMR.
• Current CPR certification if required by state.
• Current driver’s license, vehicle insurance, and dependable transportation.

Pay: $41.00 – $52.00 per hour (Average Pay Range). Final compensation is determined by experience, qualifications, and responsibilities.
