School-Based Caseworker

CHD's West Springfield clinic is seeking a Bachelors-level Case Worker to provide services to youth in the West Springfield Middle School. The partnership between CHD and the West Springfield Middle School aims to increase the connections and resources of youth who may be at‑risk for substance use and/or violent behavior. The right candidate will be inspired by the potential of bridging gaps and offering services creatively.

The role works in collaboration with a Master's-level Clinician to implement the therapeutic objectives of the treatment plan designed to address the youth's mental health, behavioral and emotional needs. This includes mentoring the youth to understand, direct, interpret, manage, and control feelings and emotional responses to situations and to assist the family to address the youth's emotional and mental health needs.

The case manager assists with the care coordination with other community providers that are involved in the youth's treatment. The case manager will collaboratively work with the clinician, youth, family and school to develop all service plans. Other responsibilities include facilitating small groups, access recreational activities, planning and carrying out community activities, and provide homework help.

Requirements:

• Must have a Bachelor's in a related field
• Have knowledge of mental illness, behavioral modifications, and parenting education
• Knowledge of basic computer skills
• Valid driver's license
• Must be able to work M-F 8am-4pm
