The Social Services Designee plays a vital role in supporting the emotional, psychosocial, and practical needs of residents and their families. This position works collaboratively with the interdisciplinary care team to ensure residents’ rights, quality of life, and successful adjustment to the facility.

• Conduct psychosocial assessments and develop individualized care plans
• Assist residents and families with admission, adjustment, and discharge planning
• Provide counseling, emotional support, and crisis intervention as needed
• Advocate for resident rights and ensure compliance with state and federal regulations
• Coordinate with families, community agencies, and external resources
• Assist with advance directives, care conferences, and resident/family meetings
• Maintain accurate and timely documentation in accordance with facility policy
• Participate in interdisciplinary team meetings and care plan conferences
• Support grievance resolution and address psychosocial concerns

• Bachelor’s degree in Social Work or a related human services field preferred but not required
• Minimum qualifications to serve as a Social Services Designee under Minnesota and federal regulations required
• Experience in long-term care or healthcare setting strongly preferred
• Knowledge of Medicare/Medicaid, resident rights, and discharge planning
• Strong communication, organization, and advocacy skills
• Ability to work independently and as part of a multidisciplinary team
