This specific Community Living Specialist - Case Manager/Certified Assessor performs functions related to case management for persons who have a disability or may be at risk for placement in a nursing/hospital/intermediate care facility, residential treatment facility, or foster care, and receive home and community based services. This position will also be a certified MnCHOICES Assessor for their caseload. This individual will perform functions related to eligibility for individuals with disabilities, developmental/intellectual disabilities using the MnCHOICES tool.
The area/population this position will work with:
Developmental/Intellectual Disabilities Rule 185: Individuals with developmental/intellectual disabilities who are receiving non‑waivered county and state‑funded services.
Minimum QualificationsNEED TO HAVE:
- Bachelor's degree in one of the following fields:
Social Work, Psychology, Sociology, Human Services, Special Education, or related Human Services field; - One year full‑time professional experience OR two years full‑time para‑professional experience in a human service agency;
- Required license(s):
Valid Driver's license. - Must meet QDDP (Qualified Developmental Disabilities Professional) or formerly known as QMRP Qualifications. Find the definition of QDDP here. Search CBSM-Glossary and then the letter Q.
NICE TO HAVE:
- Master's degree and/or experience in Social Work, Psychology, Sociology, Special Education, or related field.
- Prior professional experience in supporting children with emotional, behavioral and developmental disabilities including mental health diagnoses and/or children receiving in‑home or community based settings.
- Ability to read, write, and/or speak in a secondary language (e.g., Somali, Spanish).
