The role is a Nursing Center Social Services Specialist working with patients/residents and families/significant others to identify strengths and social, emotional, and mental health needs and provide services to meet those needs. Goal is to provide highest practical level of physical, mental, and psychosocial well‑being and quality of life. Services provided in accordance with NASW Code of Ethics and compliance with federal, state, local guidelines and regulations, company policies and procedures, and standards of care.
Responsibilities- Collaborate with interdisciplinary team to promote and protect patient/resident rights and well‑being, including educating staff on role of social services.
- Participate in development of written interdisciplinary plan of care for each patient/resident to address social service needs.
- Hold meetings with patients/resident, families, and significant others as requested and provide support and information to improve quality of life.
- Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers.
- Establish relationships and maintain contact and referral flow with community‑based agencies/services that benefit our patient/resident population.
- Bachelor’s degree in social work.
- Must possess any certifications/licensures required by State of employment to practice in long‑term care.
