The Care Home Support Team provides treatment and care for a wide range of healthcare needs for residents in Care Homes within the London Borough of Sutton. Main duties of the jobSutton Community Health and Care is looking for a dynamic, enthusiastic and innovative Band 7 Occupational Therapist with experience in Health and Social Care to complement our Care Home Support team. This role aims to support the review of resident's mobility regarding activities of daily living, with the formulation of robust plans to support the resident maintaining levels of independence where able.
To have clinical responsibility for assessment and treatment planning for people referred into the service
Will be required to work autonomously and as part of the multidisciplinary care team
The clinical caseload will comprise of adult clients with physical and cognitive difficulties within the community and/or bed-based settings
To provide supervision and leadership to other therapists and care home assistants within the team
To assist in the overall development and management of Occupational Therapy across the service
To collaborate with colleagues across own and other community teams, acute teams, GPs, social services and voluntary sector to optimise patients' journey, care, wellbeing and outcomes.

Person Specification- Diploma/Degree in Occupational Therapy
- HCP Registration
- Significant post registration experience, some of which demonstrable in acute or community setting
- Leadership and Management skills
- In absence of community experience appreciation of constraints to working in the community
- BAOT member
- Ability to demonstrate on-going CPD and the use of evidence based practice
- Ability to manage own caseload and manage time effectively
- Comprehensive knowledge of equipment
- Ability to conduct training/teaching in services
- Knowledge of working in the care home system and knowledge of Care Home Regulations
- Using EMIS clinical system
