Role:
Travel Home Health Physical Therapist responsible for providing outpatient physical therapy services in a home health setting across a multi-county area.
Responsibilities:Deliver therapy for patients with orthopedic, weakness, and cardiac rehab needs; manage a caseload of 5-6 units daily; utilize Epic EMR; collaborate with a team of PTAs; ensure patient care quality and documentation accuracy.
Qualifications & Skills:Minimum of 2 years of physical therapy experience, BLS certification, WV licensure if applicable, positive attitude towards learning Epic documentation, flexibility in coverage area, and ability to work independently with self-scheduling.
Key Duties:Administer therapy during day shifts (8am-4:30pm, Monday-Friday); provide services in patients' homes; support general orthopedic and cardiac rehab cases; participate in department orientation; adhere to dress code and cancellation policies.
