Well Be Senior Medical is seeking a Nurse Practitioner for in-home adult medicine care in Teays Valley, WV. You will perform comprehensive primary care visits in patients' homes, manage chronic diseases with longitudinal plans, and coordinate with an interdisciplinary team to ensure safe transitions of care.
Requirements include a Master’s Degree (NP) with focus on Acute Care/Adult/Geriatric, active WV NP license, and ability to travel locally.
