This is a full time, grant-funded position. It will be located primarily in Mingo County, WV, including MLIH sites at Matewan, WV and Chattaroy WV, with travel to other MLIH sites and other locations as needed.

The Community Health Worker (CHW) will play an essential role in the delivery of health services. The CHW will work as a liaison between patients, families, and the healthcare system, with the goal of improving health and well‑being of underserved populations. The CHW will assist in the provision of health education, advocacy, and resource coordination to increase access to care, reduce health disparities, and enhance health outcomes in the community.

• Outreach and Engagement:
  Engage with patient and the community to promote health services available at the health center, including preventative care and chronic disease management. Conduct community outreach through community partner and mobile unit visits, phone calls, and in‑person engagement in the community to assess health needs and provide education on health and wellness. Identify at‑risk populations, including those with barriers to accessing care, and assist with enrollment into relevant services (e.g., Medicaid, health insurance, etc.).
• Health Education and Support:
  Provide culturally appropriate health education on topics like nutrition, mental health, and chronic disease management, while helping patients navigate healthcare options and understand diagnoses and treatment plans.
• Care Coordination:
  Assist in coordinating patient care by scheduling appointments, facilitating referrals, supporting healthcare navigation, and following up to ensure continuity of care.
• Advocacy:
  Advocate for patients by ensuring access to necessary resources and supporting their understanding of rights and responsibilities within the healthcare system.
• Data Collection and Reporting:
  Maintain accurate records of patient interactions and services, track community health needs and outcomes, and participate in quality improvement activities, including data collection for health center reporting and evaluations.
• Ensure efficient virtual visit connections for patients, nursing staff, and providers, provide basic technical support, coordinate with IT for technical issues, and collaborate with Registration to schedule or reschedule telehealth appointments.
• Measure and record vital signs, prepare patient room for examination, and collect specimens as needed. Schedule and follow up on appointments, ensure lab samples are labeled and stored correctly, and assist with telemedicine and clinical staff tasks. Maintain patient confidentiality, manage supplies, and enter data into clinical systems while communicating with patients, staff, and other agencies.

This position is necessary to assure the effective and efficient delivery of clinical services. It requires a thorough knowledge of modern office practices and procedures; ability to type accurately at a fast rate, ability to deal tactfully and courteously with the public; ability to secure cooperation and establish and maintain effective working relationships with superiors, staff, and the general public; ability to accept supervision and follow instructions;

ability to perform duties with a minimum of supervision; ability to handle matters of a confidential nature, and provide support to the clinical staff completing basic medical tasks.
