Salary – Up to $48. Per hour

Bonus – Up to $4000

Serenity Hospice is searching for a Hospice RN Case Manager. This is a full-time position. Minimum of 1 year as a RN in a hospice or skilled nursing environment and 1 year as a Case Manager - preferable

In this role you will provide our patients and families exceptional end of life care that supports dignity and comfort for individuals, provide peace of mind to families, and honor cultural traditions and customs as applicable.

The ideal candidate enjoys being part of a cohesive team but is adept at working independently when needed.

This position will be responsible for providing care to a growing caseload of patients. Our Hospice RN Case Manager will identify and evaluate palliate needs of patients utilizing the nursing process with attention to pain assessment and response treatment. The ability to communicate broadly is highly desirable.

Weekly schedule: Monday to Friday

Experience: Hospice A=care: 1 year (Preferred) Skilled Nursing Facility: 1 year (Preferred)
