** As a
** Behavioral Health RN with Elara Caring**, you’ll help patients manage their health in familiar surroundings. You’ll build relationships, understand the challenges affecting their daily lives, and coordinate care around their individual needs.  This role serves
** Cleveland’s west side and nearby communities**, including
** Lakewood, Rocky River, Westlake, North Olmsted, Avon, Avon Lake, Lorain, and Elyria**.

** What You’ll Do
*** Assess patients’ physical, behavioral health, psychosocial, and environmental needs.
* Manage your assigned caseload, including assessing, planning, coordinating, providing, and evaluating nursing care.
* Reassess patients during visits and update care plans as needs change.
* Teach patients, families, and caregivers how to follow the care plan and manage symptoms.
* Communicate significant findings, changes in condition, and safety concerns to the clinical supervisor, physician, and care team.
* Coordinate with other providers and care settings to support continuity of care.
* Supervise and document behavioral health aide and LPN care according to agency policy.
* Maintain accurate clinical documentation.
* Provide support at the time of death and complete bereavement assessments when applicable.  
** What You’ll Need
*** Current, unrestricted RN license valid in Ohio.
* At least
** 1 year of nursing experience in a clinical care setting**.
* Ability and willingness to travel throughout the assigned territory, with approximately
** 50% travel**.
* A dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.  
** Preferred Qualifications
*** Associate or bachelor’s degree in nursing.
* Home health or hospice experience.
* Homecare Homebase experience.
