PACE Southeast Michigan is seeking a Masters Social Worker to serve participants in the Westland area. You will assess needs, coordinate services, and deliver participant-centered social work in home settings, focusing on safety, independence, and dignity.
Under the Social Work Manager, you will conduct bio-psychosocial assessments, develop treatment plans, provide counseling (MI and CBT), and support care coordination with the IDT.
