PASCO is currently seeking acompassionate and dedicated RN to provide personalized one-on-one care for a male client in his early 20'swho lives in Westminster. Client requires respiratory and GB cares.

This position will be needed on a part or full-time basis with a schedule of any consecutive days M-F working a10-hour (8pm-6am) OR 12 hour (8pm - 8am) shift. Client has x2 hypoallergenic dogs and selected candidate must be ok with this.

POSITION

Private Duty Nurse (PDN)

POSITION PURPOSE

Provide direct, comprehensive nursing services for a single male client on an hourly basis overnight. Additionally, this role will provide client education, complete and submit all required documentation, and may supervise or direct other assigned care providers.

KEY DUTIES & RESPONSIBILITIES

• Deliver skilled nursing care according to physician orders and industry standards.

• Conduct holistic client assessments and implement appropriate care procedures.

• Support clients in achieving care goals and adapt plans as their needs change.

• Build and maintain positive relationships with clients and their support systems.

• Coordinate care with the RN Case Manager, family, physicians, and other staff.

• Maintain accurate in-home charts, medication records, and required documentation.

• Report lab results and health changes to the care team promptly.

• Participate in case conferences and ensure alignment with individualized care plans.

• Supervise and guide LPNs, CNAs, and other care providers, maintaining quality standards.

• Educate clients and caregivers, offering referrals to community resources when needed.

• Adhere to HIPAA, PASCO policies, and state and federal regulations.

• Communicate clinical updates and attend meetings for ongoing professional development.

• Ensure reliable transportation, a valid Colorado driver’s license, and active insurance.

EXPERIENCE

• 1-3+ years experience practicing as a PDN providing in-home care (preferred)

• Documented experience and skill set matching client/family needs

• Home Health nursing experience (highly preferred)

• Active and current drivers license/insurance

EDUCATION/LICENSES

Current license to practice as a Registered Nurse in the state of Colorado (confirmed through DORA)

• Current CPR Certification

KNOWLEDGE/SKILLS/STRENGTHS

• Remain alert and attentive during overnight hours while delivering care to client

• Clinical Judgment:
  Ability to independently collect and interpret data about a client, arrive at a conclusion and identify the appropriate action. This involves problem solving, decision making and critical thinking rooted in clinical knowledge and experience.

  Ability to establish and maintain relationships with clients, and provide professional appropriate, compassionate and empathetic care to clients

• Knowledge of State, Federal and Licensing rules and regulations for care delivery and documentation by Home Care Agencies

• Knowledge of unique issues related to care delivery for people with a variety of disabilities

• Ability to assess data reflective of the client’s status

• Ability to communicate effectively both orally and in writing

• Ability to operate a computer, familiarity with Microsoft products, specifically Outlook, and ability to receive and process work in an Electronic Health Record (EHR) system

PHYSICAL REQUIREMENTS
