You will coordinate interdisciplinary planning, discharge planning, and patient/family teaching for post-hospitalization, including accessing community resources.
Eligible candidates hold an active RN license, current BLS, and ACLS within 90 days. Inpatient medical-surgical experience is preferred but not required.
