Overview
PHYSICIAN-HOSPICE role at Life Bridge Health (Bridging Life, a Life Bridge Health partner). Part-Time Hospice Physician (20 hours per week / 0.5 FTE) to work with our Medical Director in caring for hospice patients. This position has the potential to work on-site at Bridging Life facilities, including Dove House (Westminster), Northwest Hospital (Randolph/ Randallstown), or Sinai Hospital (Baltimore).
For more than 35 years, Bridging Life has delivered compassionate care to patients and their families in our community, expanding across the Life Bridge Health continuum to serve people living with serious illness in palliative care, home hospice care, inpatient hospice care, and grief support.
Responsibilities- Attend and actively participate in weekly Interdisciplinary Group (IDG) meetings, providing clinical leadership and medical expertise to IDG members.
- Certify and recertify patient eligibility for hospice care, considering diagnosis, comorbidities, other relevant information, and documentation of disease progression.
- Complete required narrative for certificates of terminal illness.
- Before a patient’s third and subsequent re-certifications, ensure clinical findings support continued hospice eligibility.
- Assist in developing, implementing, and revising initial and ongoing comprehensive plans of care (POC) for each patient, including pain and symptom management and establishing measurable outcomes aligned with patient rights.
- Provide accurate and timely documentation of all patient care services.
- Assume medical care responsibilities for hospice patients in collaboration with the attending physician and IDG, including prescribing and signing death certificates as needed.
- Review patient medications for effectiveness, side effects, interactions, and monitoring requirements; assist IDG in defining meaningful outcomes for interventions and medications.
- Participate in on-call coverage with the Medical Director and other providers, including potential overnight and weekend coverage and bedside rounding on weekends/holidays as needed.
- Advise management and staff on complex POC issues and resolve conflicting patient care decisions with the attending physician as needed.
- Optimize comfort and dignity for patients in alignment with patient and family goals.
- Assist the Medical Director in managing inpatient and outpatient palliative care programs; clinical or administrative work with palliative care teams may be available.
- Serve as physician designee in the absence of the Medical Director.
- Perform other reasonable duties as assigned by the Medical Director or designee.

- Salary range: $130,000 - $140,400 (based on experience)
