Work schedule:
Monday - Friday with flexible schedules.
As a Registered Dietitian at HCSG, you will manage clinical nutritional services at a long‑term care facility, positively impacting residents and the community.
Responsibilities- Assess residents’ clinical nutrition needs and develop and document individualized plans in the electronic medical record.
- Implement and evaluate care plans based on clinical findings, lab results, weight trends, intake, and comorbidities.
- Provide nutrition education and counseling to residents, families, and staff.
- Collaborate with interdisciplinary teams to improve health and well‑being.
- Ensure compliance with CMS, state, and federal regulations and company standards.
- Provide coverage in multiple buildings as needed.
- Perform all other duties as assigned.
- Registered Dietitian by the Commission on Dietetic Registration.
- Hold a valid certification/license in good standing within the state of practice.
- Knowledge of state survey processes and nutrition care regulations (preferred).
- Strong organizational and communication skills.
- Positive attitude and effective communication with residents, staff, administration, and leadership.
- Compliance with COVID‑19 vaccination policies as required.
- Valid driver’s license if a multi‑site dietitian.
- RD‑Eligible and provisional state licensure accepted in certain locations.
