Discharge and Flow Coordinator
University Hospitals Bristol and Weston NHS Foundation Trust (UHBW) invites applications for a Discharge and Flow Coordinator – a key member of the Transfer of Care Hub supporting patients’ discharge planning from admission through to community care. The role focuses on working collaboratively across multidisciplinary teams to ensure patients have a smooth transition home.

Main duties of the job- Act as the first point of assistance for internal and external enquiries regarding patient discharge.
- Facilitate and support early communication between hospital staff, patients, and families related to discharge planning.
- Manage patient communications via telephone and Trust systems, creating letters and emails.
- Serve as a point of contact for queries from clinical and non‑clinical staff.
- Liaise with clinical and non‑clinical teams via telephone and email.
- Escalate patient queries, capacity problems and general issues to line manager or relevant clinical staff as appropriate.
- Support ward‑based colleagues with patient discharge plans, including managing expectations, arranging practical arrangements, booking transport, ensuring TTA’s and discharge summaries are completed, checking equipment, and liaising with onward care providers.
The position is full time, Monday to Friday, 8:00 a.m. – 4:00 p.m.; weekend working will be on a rota basis.
Qualifications and Training- Educated to GCSE band 4/5 or equivalent in English and Maths.
- Business and Administration Level 3 NVQ or equivalent experience.
Skills and Abilities
- Effective and polite demeanour when dealing with people face‑to‑face and on the phone.
- Ability to work under pressure and meet deadlines while maintaining a high standard of accuracy.
- Strong organisational and multi‑tasking skills with the capacity to prioritise workload.
- Excellent communication and engagement skills.
- Ability to adapt to changing working environments and to work both independently and as part of a team.
- Confidence in handling difficult conversations with patients, relatives, and carers.
- Good IT skills including Microsoft Office and experience with Patient Administration Systems.
- Knowledge of admission policies and procedures.
- Clerical or administrative experience, including filing and/or reception duties.
- Experience dealing with the public, both on the telephone and in person.
- Previous NHS experience.
- Experience using a Patient Administration System.
