This is an exciting opportunity to join our Integrated Recovery & Social Care Mental Health Service in North Somerset, supporting adults of working age with complex mental health needs. You'll play a vital role in helping people live meaningful, fulfilling lives while developing specialist knowledge and practical experience across key areas of adult social care legislation and mental health practice.

We are looking for a committed and skilled Adult Social Care Worker to join our integrated Recovery & Social Care Mental Health Service in North Somerset. As an Adult Social Care Worker, you will work with adults experiencing severe and enduring mental illness, supporting them to maximise independence, maintain community connections, and achieve positive outcomes.

This is a varied and rewarding role where no two days are the same. You will hold your own caseload whilst receiving support, guidance, and opportunities to learn from experienced colleagues.

You will:

• Undertake strengths-based assessments of care and support needs under the Care Act 2014.
• Complete reviews of care and support plans to ensure services remain person-centered, outcome-focused, and responsive to changing needs.
• Work closely with providers, service users, carers, and families to promote high-quality care and support.
• Support Social Workers with Mental Capacity Assessments, Best Interest Decisions, and safeguarding enquiries.
• Contribute to Section 117 Aftercare Planning and help ensure statutory responsibilities are fulfilled.
• Review commissioned services and packages of care, identifying opportunities to improve outcomes and promote independence.
• Work collaboratively with a wide range of professionals and partner agencies to deliver coordinated care.
• Respond proactively to emerging risks and prevent breakdowns in support wherever possible.

The role includes working across community settings, service users' homes, and office-based environments.

• To provide strengths based Assessments of care and support needs and risks under the Care Act 2014 and S117 MHA, working collaboratively with service users and carers.
• Identify and respond to safeguarding concerns, contributing to both NHS and Local Authority enquiries.
• Support Mental Capacity Assessments and Best Interest decision-making alongside senior staff.
• Deliver a full range of statutory and social care tasks, offering specialist knowledge within the integrated team.
• Complete timely reviews in line with North Somerset policies, procedures, and statutory frameworks.
