  • Salary Range: $70,000 - $92,000
Qualifications
  • Must hold an active Nursing license in the State where the position is based.
  • Preferably 2+ years of nursing experience, with a focus on geriatrics.
  • New nurses and LPNs are encouraged to apply; we offer an exceptional training program.
  • Nursing degree from an accredited institution.
  • Excellent bedside manners and exceptional communication skills.
  • Working knowledge of healthcare laws and regulations.
  • Maintain current BLS CPR certification.
  • Possess a valid driver's license and own a reliable, insured vehicle.
Responsibilities
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed by physicians.
  • Conduct initial assessments of patients and families to determine home care needs.
  • Perform thorough physical assessments and gather comprehensive medical histories.
  • Develop and revise individualized plans of care based on patient status and evolving needs.
  • Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
  • Responsively address inquiries and concerns from patients, caregivers, and medical teams.
  • Provide counseling and support to patients and caregivers before, during, and after treatment.
  • Supervise home health aides as required.
  • Identify discharge planning needs and proactively implement them as part of the care plan development.
