Shift: 1st Shift – 16 hours Saturday & Sunday, 7:00 am – 3:00 pm
Hourly: $19.39 (Weekend differential $20.39)
Scope of Service:
Assists individuals with daily personal routines, provides prompts for daily living skills, skill‑building activities and participation in groups. Provides companionship, monitors medical concerns with licensed or visiting nursing personnel, reports concerns, ensures safe living spaces, leads group activities and outings, collaborates with nursing management and coworkers to deliver quality services, performs other duties as assigned.
- Establishes rapport and maintains professional ethics and boundaries with clients.
- Enhances client independence by engaging, assessing and providing modalities of teaching and assistance (e.g., modeling, prompting, hands‑on). Provides hands‑on support for activities of daily living such as personal care/hygiene, housekeeping, laundry, food management, budgeting, health & safety, leisure and other healthy living activities.
- Facilitates/co‑facilitates psycho‑educational groups; engages clients in community integration and social recreation.
- Provides transportation to appointments; educates clients on travel planning and public/private transportation.
- Conducts regular apartment/room inspections and assists clients in maintaining cleanliness.
- Conducts ongoing rounds checks for safety; administers breathalyzer and urinalysis tests; documents shift logs electronically; develops relationships with providers, families and natural supports.
- Responds to crisis situations with appropriate interventions; supervises medication self‑administration; attends monthly staff meetings, clinical rounds and supervision sessions.
- Maintains a clean, well‑organized work environment; ensures all program egresses and walkways are clear.
- Notifies supervisor of maintenance or safety issues and generates work orders; participates in fire/drill and emergency preparedness plan reviews.
- Participates in Food Bank activities as assigned; remains awake during all shifts to maintain client/facility safety.
- Adheres to policies in staff handbook, program operations manual and CARF standards.
- Uses HIPAA-compliant practices; obtains necessary releases of information.
- Models culturally competent behavior; maintains accurate electronic records and case files.
- Completes all required trainings and attends agency-sponsored in‑services, conferences and meetings for professional growth.
- High School Diploma or equivalent with at least one year related healthcare experience (CNA or equivalent certification acceptable).
- Prior experience with individuals with severe mental illness, intellectual disabilities, or addiction.
- Knowledge of community support systems and resources preferred.
- Valid Connecticut driver’s license with clean record; reliable transportation for scattered site and non‑24‑hr. positions.
- CPR/First Aid certification at orientation; must maintain ongoing certification.
- Proficient computer skills (Microsoft Office, electronic records).
