Join our team as an Embedded Nurse Practitioner or Physician Assistant. In this impactful role, you will conduct annual wellness exams and vital screenings that enhance the quality of patient care. You will perform thorough health assessments, conduct non-invasive screenings, and offer personalized recommendations for disease management. Your contributions to patient education and health promotion will make a real difference in our community.

Open to Nurse Practitioners and Physician Assistants. Rotation between Mystic, Waterford, Plainfield, Norwich, and New London. 100% Outpatient Primary Care.
- Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
- Consult with physicians and care team members to develop plans for further evaluation, e.g., referral to specialists.
- Provide appropriate treatment within scope of practice, identify issues outside scope, and refer appropriately to physician.

Unrestricted State of Connecticut Medical License (or eligible) - Training:
Nurse Practitioner (Family, Adult/Geri Nurse Practitioner) or Physician Assistant - Experience:
2+ years’ experience in a primary care or relevant outpatient setting
