You will perform thorough health assessments, conduct non-invasive screenings, and offer personalized recommendations for disease management. Open to Nurse Practitioners and Physician Assistants. Rotation between Mystic, Waterford, Plainfield, Norwich, and New London. 100% Outpatient Primary Care.

• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
• Consult with physicians and care team members to develop plans for further evaluation, e.g., referral to specialists.
• Provide appropriate treatment within scope of practice, identify issues outside scope, and refer appropriately to physician.

• Licensure:
  Unrestricted State of Connecticut Medical License (or eligible)
• Training:
  Nurse Practitioner (Family, Adult/Geri Nurse Practitioner) or Physician Assistant
• Experience:
  
  2+ years’ experience in a primary care or relevant outpatient setting
