Position: Health Inequalities Nurse - Children & Young People

An exciting opportunity has arisen for a senior nurse to work across Weymouth and Portland to support development of services for Children and Young People, with a particular focus on inequalities. This role is ideally suited to a senior nurse or ACP, and experience of working with this population is essential. It is part time hours 16 hours per week. It is initially offered as a fixed term 12 month contract, but if successful then there is scope to become permanent.

You join a team including a lead GP for CYP and a Lead ACP for Inequalities - with strong administrative support.

Main duties of the job

You will beworking for all of the Practices within the Weymouth and Portland PCN - The Bridges Medical Centre, Cross Road Surgery, Wyke Regis & Lanehouse Medical Practice, Royal Manor Healthcare, Weymouth Bay Medical Practice.

The overall aims of the role are to:

With support, identify the inequalities in health care provision andaccess to GP services for children and young people in the Weymouth and Portland PCN. Plan and carry out practical interventions to reduce or removebarriers leading to an increase in access to health care in a variety of wayswhich could be unique and creative.

Job responsibilities

Job Purpose/ Role and Clinical and Managerial Responsibilities

  • An integral member of the PCN team
  • An integral member of the local CYP Multi Disciplinary Team (MDT)
  • Represent and champion the health needs of local CYP
  • Contribute tothe development of the role and new ways of working
  • Keep abreast oflocal and national data and policy pertaining to CYP and disseminate and actionas appropriate
  • Work with CYP,carers, families and people who work with CYP to survey and understand barriers to accessing healthcare and needs, usethis knowledge to shape services
  • Create links,lines of communications and working relationships with local relevant teams and services including but not exhaustive: child health immunisation team, healthvisitors ,school nurses, schools, public health teams, youth services, GP practices, asylum seeker and refugee support teams, Libraries, Citizen advice, local family Hubsand foodbanks.
  • Create and maintain ways and methods for GP practices to communicate with CYP which may benew and innovative and based on the CYP voice
  • Record andmonitor the activities and impact of the role and share this information withthe PCN board on a quarterly basis
  • Ensure effective planning and division of time to carry out various areas of work

Key areas ofWork

PCN GP practices

Lead a localnetwork for GP practice CYP champions to support practices to ensure services are CYP friendly and meet need, to include but not exhaustive: appointmentsystems, information on practice websites and appropriate information such asconsent and confidentiality, access to appointments and use of IT resources.

On request audit

GP practices care provision for CYP and provide advice for improvements

Care Leavers support

Work with care leavers support teams within social services to supportinformation on access to general practice services being provided to young careleavers

Ensure GP practices provide any required reasonable adjustments for individual care leavers requiring additional support

Support individuals to access GP services as required

Transition between child and adult services

Provide information and links for young people and their carers with aLearning Disability on GP services and add to learning disability registers as required

Provide information and education on GP and other health services toyoung people close to turning 18 years of age.

Education

Work with local schools, colleges and youth services to provide health information and education sessions in schools to promote general health adviceand how to access general practice services. Tailor the education sessions tosuit the needs and ensure education on confidentiality and consent.
