Job Description
  • 0.03 FTE
Hours / Schedule

IHR (In House Registry) to cover PTO, leave, or schedule gaps. Expectation 192 hours in a year, which includes 4 weekend days per quarter and 2 holidays. Monday–Friday weekday availability is also preferred.

  • Not a benefited position
  • Licensed physical therapist who complies with the Code of Ethics / Standards of Practice as outlined by their professional organization and by the State of Illinois.
  • Position includes direct patient care, teaching, participation on appropriate committees, and departmental process improvement.
  • Serves as a role model for all therapists and clinical interns in all aspects of professional, clinical, communication, education, and leadership activities.
Responsibilities
  • Demonstrate the four NM values of teamwork, patients first, integrity, and excellence in daily work and interactions.
  • Present a friendly, approachable, professional demeanor and appearance.
  • Provide accurate information and timely updates to patients and customers; address questions and concerns promptly or identify the appropriate person and resources.
  • Use effective service recovery skills to solve problems or service breakdowns when they occur.
  • Maintain primary responsibility over patient plan of care through timely and consistent collaboration with treating PT/PA.
  • Demonstrate teamwork by helping co‑workers within and across departments; communicate effectively, respect diverse opinions, and acknowledge assistance and contributions of others.
  • Use organizational and unit/department resources efficiently.
  • Manage work schedule efficiently, completing tasks and assignments on time.
  • Contribute to opportunities and processes for continuous improvement.
  • Participate in efforts to reduce costs, streamline work processes, and improve services provided.
  • Demonstrate creative problem solving and adapt treatment to special circumstances based on team feedback and documentation.
  • Apply appropriate evaluation and treatment modalities, equipment, and protocols; perform re‑evaluations per department guidelines and adjust the treatment plan accordingly.
  • Assume complete responsibility for physical therapist patient management from initial contact to discharge for functional progress, safety, and treatment outcome.
  • Adhere to department policy regarding evaluations, progress notes, and discharge summaries; document all notes in electronic medical record (EMR).
  • Complete daily notes that reflect patient’s response to treatment, performance, and assessment relative to the plan of care.
  • Provide educational materials and methods appropriate to patient’s level of understanding and motivation.
  • Participate in committees, department processes, and special projects in accordance with clinical expertise and performance plan.
  • Develop and maintain professional relationships with members of the health‑care team (e.g., physician, nurse, case manager); participate in internal and external marketing per department expectations.
  • Create a culture of respectful communication by listening, seeking team input, and acknowledging contributions.
  • Accept constructive feedback and make timely changes as evidenced by coaching forms and team feedback.
  • Proactively complete non‑patient care responsibilities within established timelines (coverage notes, PTO/schedule change requests, bonus pay follow‑ups).
  • Contribute to educational efforts of the department/clinic (e.g., in‑services, journal clubs, new hire orientation).
