Overview
We are seeking a compassionate nurse to support our Independent Living residents with a predictable Monday-Friday schedule, light nursing duties, and a focus on wellness and connection.
Hours7:00am-3:30pm, Monday-Friday
Responsibilities- Evaluates the physical, emotional, and social needs of residents.
- Maintains or improves independence through directing and educating residents and advising on adaptive equipment.
- Assesses residents’ cognitive and physical abilities and recommends adaptations to reduce safety barriers.
- Assists in the orientation of new residents and their adjustment to community living.
- Maintains accurate and complete reports, records, physician orders, care plans, and progress notes while ensuring confidentiality.
- Communicates with physicians, families, and staff regarding resident status.
- Provides support to residents and families dealing with aging and illness.
- Offers timely updates and addresses questions promptly or refers to appropriate parties/resources.
- Encourages residents to participate in social and creative activities.
- Coordinates services for residents being admitted to or discharged from Assisted Living, Health Center, and hospital.
- Monitors infections and accidents and notifies families and physicians appropriately.
- Monitors private duty personnel and coordinates care with agency representatives.
- Responds urgently to emergent health issues of residents.
- Minimum two years of clinical or case management experience with elderly population (strongly preferred).
- Active registered nurse or licensed practical nurse license in Illinois.
- Current CPR/BLS certificate.
- Strong English language skills—reading, writing, speaking.
- Walk long distances for 50% of the workday.
- Lift and carry up to 30 lbs.
- Push a wheelchair occupied by a resident weighing up to 200 lbs independently.
- Respond safely to emergent situations throughout the entire building.
- Up to 21 days per year of PTO.
