Position: Supportive Housing Case Manager

Supportive Housing Case Manager – Du Page Pads

We are looking for a dedicated, client‑focused professional to support individuals and families experiencing housing challenges. This role provides advocacy, education, and referral services, helping clients achieve greater independence while coordinating with community providers.

Base Pay Range

$22.00/hr – $23.00/hr

Job Summary

This position is responsible for providing advocacy, education, and referral services to program participants and engaging collaboratively with community providers to achieve client goals. Work includes case management, goal planning, documentation, and community liaison duties.

Primary Responsibilities
  • Operate within the Du Page Pads  Core Values, supporting care in accordance with the Trauma Informed Policy and Practices of the Agency.
  • Manage a case load of clients and provide skill training to meet assessed needs with the intent to transition clients to greater independence.
  • Act as client advocate to facilitate access to resources and services.
  • Work collaboratively with providers to achieve client goals.
  • Develop and maintain client goal plans, annual assessments, and completion or coordination of required forms.
  • Serve as liaison between community resources and clients.
  • Maintain documentation expectations with client contact, monthly progress reports, accurate confidential case files, and contractual paperwork.
  • Maintain collaborative relationships with co‑workers to effectively support clients.
  • Provide housing support groups and workshops collaboratively with co‑workers to help support participants’ goals.
  • Maintain confidentiality of privileged information and adhere to client privacy laws, demonstrating sensitivity to other ethnic groups and cultures.
  • Act as housing liaison between clients and landlords, collecting rents, attending housing issues, coordinating tenant moves in and out of apartments.
  • Drive program participants to activities or appointments on a regular basis.
  • Participate in client staffing supervision and team meetings, agency meetings, and required trainings.
  • Adhere to department guidelines for attendance and punctuality.
  • Provide phone coverage and/or response on weekend days and overnights.
  • Flexibility in scheduling.
  • Other duties as assigned by supervisor.

Qualifications
  • Bachelor’s degree in Social Work or related field, or 3 years’ experience in a related field.
  • Experience in housing‑related activities is a plus.
  • Certification in substance use is a plus.
Additional Requirements
  • Flexible hours required.
  • Ability to lift 10 pounds.
  • Provide own transportation and possess a valid Illinois Driver’s License.
Classification Status

Non‑Exempt Full‑time.
