Social Worker — LBSW or LMSW

As a Social Worker at Montcare, you will play a vital role in supporting our mission to provide compassionate and individualized care to patients across our rehabilitation and skilled nursing centers. You will work closely with our multidisciplinary teams to facilitate patient-centered care, promote independence, address psychosocial needs, and ensure a seamless transition to the next level of care.
Responsibilities- Assess patients' psychosocial needs and develop individualized care plans in collaboration with patients, families, and the interdisciplinary team.
- Provide supportive counseling and emotional support to patients, families, and caregivers.
- Address patients' social, emotional, environmental, and adjustment needs throughout their stay.
- Facilitate patient education, discharge planning, and coordination of services with community resources.
- Assist patients and families with navigating available healthcare, social service, and community resources.
- Collaborate with physicians, nursing staff, therapists, and other healthcare professionals to identify and address barriers to patient care and successful discharge.
- Participate in care plan meetings and interdisciplinary team discussions.
- Advocate for patients' needs, preferences, rights, and well-being.
- Maintain accurate, timely, and confidential documentation of patient assessments, interventions, and care plans.
- Support patients and families through transitions of care and facilitate appropriate referrals to community and healthcare resources.
- Comply with all applicable federal, state, and facility regulations, policies, and standards of practice.
- Bachelor of Social Work (BSW) or Master of Social Work (MSW) from an accredited social work program.
- Current and valid LBSW (Licensed Bachelor Social Worker) or LMSW (Licensed Master Social Worker) license.
- Previous experience in healthcare, rehabilitation, skilled nursing, long-term care, or a related setting is preferred.
- Strong communication, interpersonal, organizational, and documentation skills.
- Demonstrated ability to work effectively as part of an interdisciplinary healthcare team.
- Compassionate, patient-centered approach to working with patients and families.
- Ability to maintain confidentiality and comply with applicable healthcare regulations and facility policies.
