MINIMUM QUALIFICATIONS

1. Certified Diabetes Care and Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education
1. One (1) year experience in health and fitness

PREFERRED QUALIFICATIONS

1. ASCM, Clinical Exercise Physiology Certified

CORE DUTIES AND RESPONSIBILITIES

Identifies education needs of the community and those with diabetes.

Attends diabetes advisory committee meetings as assigned.

Performs patient assessment, delivers education and evaluates patient’s obtainment of goals.

Assists in maintaining the American Diabetes Association certification as applicable.

Participates in community outreach and educational support.

Advocates on behalf of the patient and families.

Communicates with primary care providers, members of the multi-disciplinary team relevant questions concerns, and patient outcomes.

Participates in performance improvement activities to assure quality care.

Provides administrative support and coordinates the management and operations of the Diabetic Institute

Leads the planning, implementation and evaluation of the Outpatient Diabetes and Nutrition Education Program.

Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.

PHYSICAL REQUIREMENTS

Ability to lift 25 pounds.

Ability to push/pull 100lbs.

Ability to stand for several hours per day.

WORKING ENVIRONMENT

SKILLS AND ABILITIES

Computer skills, particularly Excel and Word.

Ability to understand written and oral communication.
