Position: CADI/BI & DD Waiver Case Manager

The role supports individuals on the CADI/BI and DD waivers, serving as an advocate and ensuring they access resources and services within the community. Ideal candidates demonstrate compassion, strong organizational skills, and a commitment to client success, with experience working with individuals who have developmental or intellectual disabilities. Excellent documentation and case‑noting skills are essential.

Responsibilities
  • Provide care coordination, monitor client health and safety needs, and ensure services effectively meet client goals.
  • Intervene as needed to support clients and resolve service gaps.
  • Serve as a consistent advocate and connection to resources.
Caseloads
  • CADI/BI Waiver Case Managers:
    Average caseload about 35 clients (primarily adults, some children). Meet clients 2–3 times per year.
  • DD Waiver & Non‑Waiver Case Managers:
    Average caseload about 43 clients (mix of children and adults). Meet clients 2–3 times per year.
Qualifications
  • DD Case Managers must have one of the following:
    • Bachelor’s degree in Social Work, Special Education, Psychology, Nursing, Human Services, or a related field;
      Social Work licensure required if degree is in Social Work.
    • One year of experience or a QDDP qualification in the education or treatment of persons with developmental disabilities.
    • Completion of at least one course specifically focused on developmental disabilities (Fraser can provide this course free of charge).
    • Valid Minnesota Driver’s License with acceptable record.
    • Ability to pass a DHS background study.
    • Commitment to promoting diversity, multiculturalism, and inclusion with culturally responsive practice and self‑awareness.
  • CADI/BI Case Managers must have one of the following:
    • Bachelor’s degree in Social Work, Psychology, Sociology, or a closely related field;
      Social Work licensure required if degree is in Social Work.
    • Bachelor’s degree in any field and one year of experience as a social worker, case manager, or care coordinator in a public or private social service agency.

Location, Schedule & Pay
  • Monday through Friday during standard business hours; fully remote after first 3‑6 months of onboarding is complete.
  • Fraser provides services in Dakota, Hennepin, and Ramsey County.
  • To be considered for this role, you must reside in Minnesota / Twin Cities Metro Area.
  • Travel to meet with clients is required; client meetings may occur 2‑3 times per week.
  • Starting pay $50,000 to $52,000 per year depending on qualifications + $1,500 hiring bonus (external candidates only).
