Base Pay: $38,000 – $40,000 per year.

Purpose of the Role

Grow the agency’s client base through relationship building in the community and personal visit sites. Identify persons who currently do not have benefits or an agency affiliation. Through a one‑on‑one meeting, determine if the person can benefit from CHOICE such that their lives become stable, recovery‑centric and interactive.

Essential Functions of the Role
  • Identify and build relationships with organizations and individuals in settings such as hospitals, shelters, partner agencies, medical care clinics, mental health providers, soup kitchens, food pantries, and other community‑based groups, promoting awareness of CHOICE, its mission, and services.
  • Meet one‑on‑one with prospective clients and screen for eligibility.
  • Review potential clients with Outreach Supervisor for final approval.
  • For eligible individuals, remain in close partnership throughout the enrollment process until intake is completed.
  • Act as a support partner, assisting them with essential care relationships through enrollment completion.
  • Proactively report trends, relationships, and developments to the Outreach Supervisor; track activity, clients, and outcomes.
  • Perform special projects and other duties as assigned.
Qualifications for this Role
  • Direct experience with a mental health condition or disability, or a family member with one, as a peer organization.
  • Ability to empathize with clients.
  • Tenacity and passion, balancing objectivity with empathy.
  • Proven effectiveness in engaging staff and individuals in shelters, soup kitchens, and similar environments.
  • Bachelor’s degree required;
    Associate’s degree accepted with experience.
  • Fluency in English and Spanish (bilingual).

Monday – Friday, 9:00 AM – 5:00 PM
