• Provide direct patient care to patients in our rehabilitative setting, including medication administration, wound care, and vital sign monitoring

• Develop and implement personalized care plans in collaboration with healthcare teams and patients

• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions and optimal outcomes

• Participate in multidisciplinary team meetings to discuss patient progress, challenges, and goals

• Maintain accurate and up-to-date patient records, adhering to HIPAA guidelines and standards
