Outreach Specialist

Position: Connective Outreach Specialist Role
Drive positive change as an Outreach Specialist with Connective, a recognized non-profit organization. Focus on delivering compassionate support to homeless service users through effective outreach programs.

In this role, the Outreach Worker will coordinate support for individuals facing homelessness, ensuring they are connected to vital community resources. You'll provide advocacy, develop personalized service plans, and maintain ongoing communication with service users and community partners. Together, we can create pathways for long-term stability and health.

Key Responsibilities:

• Analyze intake and develop tailored support strategies

• Provide advocacy and case management services

• Collaborate with service users to set realistic goals

• Ensure access to essential services and resources

• Maintain thorough records and contribute to team discussions

Requirements:

• Degree or diploma in social sciences or related fields

• At least 1 year of experience with marginalized populations

• Strong crisis intervention skills

• Familiarity with local community resources

• Certification in First Aid and Food Safe Level 1
