Clinical Pharmacist

Position: Clinical Pharmacist Role
Join Fraser Health as a Clinical Pharmacist, enhancing patient care at the White Rock Primary Care Clinic in BC. Engage in direct patient interactions and medication management while collaborating in a team environment.

This full-time position involves providing critical clinical services and ensuring comprehensive medication therapy management. You will conduct patient assessments, communicate effectively, and develop individualized care plans in partnership with healthcare professionals. Your strong clinical background and motivation will contribute to improved patient outcomes.

Key Responsibilities:

• Consult with patients and the healthcare team on drug therapies

• Implement and monitor medication management plans

• Document patient interactions accurately in health records

• Participate in safety mechanisms and reporting programs

• Provide education and training to pharmacy students and residents

Requirements:

• Registration as a Pharmacist with the College of Pharmacists of British Columbia

• Minimum of two years in relevant patient care

• Valid Class 5 driver’s license for work purposes

• Excellent negotiation and communication abilities

• Passionate about patient advocacy and safety
