Elevate community care as a Relief Full Time Community Health Nurse with Fraser Health in White Rock, B.C. Manage client health needs and collaborate on community health initiatives.

Fraser Health is seeking a Relief Full Time Community Health Nurse to join the South Fraser team until approximately April 2027. This vital role requires independent assessment of client needs and the facilitation of healthcare transitions, while contributing to community well-being. You will work collaboratively with an interprofessional team to create and implement effective care plans tailored for each client.

Key Responsibilities:

• Develop and implement individualized client care strategies

• Conduct assessments and provide therapeutic interventions

• Collaborate with healthcare teams for enhanced client care

• Maintain thorough clinical records and documentation

• Educate clients on self-management and health promotion

Requirements:

• Registered Nurse license with BCCNM

• Recent experience in geriatric and chronic health care

• Vehicle access for local travel

• Strong problem-solving and assessment skills

• Ability to prioritize and work dynamically
