The Registered Nurse (RN) or Licensed Practical Nurse(LPN) Caregiver provides skilled nursing care and personal support services to a patient in the home setting with complex medical needs, including tracheostomy, ventilator, and PEG tube care. Working collaboratively with the clinical team, the RN/LPN delivers high-quality patient-centered care, monitors patient status, administers treatments as ordered, and assists with activities of daily living to promote the patient's comfort, safety, dignity, and overall well-being during weekend shifts.###

Shifts:

*** Monday though Friday 11p-7aPerfect for nurses seeking consistent weekend shifts while providing meaningful, one-on-one patient care.###  What You'll Do
* Provide skilled nursing care for a patient with a tracheostomy, ventilator, and PEG tube.
* Monitor and assess the patient's physical, emotional, and clinical condition.
* Execute the patient care plan and communicate effectively with the RN Case Manager.
* Administer medications and treatments as prescribed and authorized.
* Assist with tracheostomy care, ventilator monitoring, and enteral feeding support.
* Provide personal care, including bathing, grooming, dressing, toileting, and hygiene assistance.
* Assist with mobility, positioning, transfers, and use of medical equipment, including a Hoyer Lift.
* Prepare meals and assist with eating and hydration as appropriate.
* Follow nutritional guidelines and care instructions established for the patient.
* Perform light housekeeping duties within the patient's living area.
* Help maintain a clean, safe, and comfortable home environment.
* Accompany the patient to approved medical appointments and community outings as authorized.
* Observe and report changes in condition promptly to the care team.
* Complete accurate and timely documentation using the Agency-approved Electronic Visit Verification system.
* Demonstrate accountability to assigned schedules and communicate scheduling needs to the Staffing Coordinator.###  What Makes You a Great Fit We are looking for someone who:
