Complex Care Coordinator

As a complex care coordinator
, you will be planning and rostering care staff into shifts and ensure that the care workers and nurses that are responsible for delivering care and support on a day-to-day basis attend their shifts as planned.

• The care coordinator will organise rotas a month in advance for the complex care clients.
• Building and developing strong relationships with staff members (carers and nurses)

• Obtaining staff availability
• Communicate with staff and clients in a respectful, professional, and friendly manner.
• On call responsibility

• You will have strong communication skills both over the phone and via written communication.
• You will be a problem solver, quick thinker and solution driven.
• You will have previous coordination experience either in complex care or domiciliary care.
• You will have the right to work in the UK and a full UK driving licence.

• Salary of £30,000
• Currently the hours are Mon to Fri 9am – 5pm, plus on call is 1 in 5 weekends (this could reduce in time) and 1 evening / morning per week (1700 – 2300/0600 – 0900). However, this may change to a different schedule.
• The role is office based (outskirts of Whitstable), with occasional travel in Kent to meet clients / families.
