Family Support Worker

Family Preservation Services (FPS) provides intensive in‑home services and case management services to families who have one or more children at risk of out‑of‑home placement or who will be at risk at birth.

• Provides for the safety, stability and well‑being of children and families during the referral period.
• Delivers and coordinates culturally competent therapeutic and case management services to children and families.
• Facilitates family meetings within required time frames.
• Engages and works in partnership with families to complete assessments and develop case plan goals and objectives.
• Responds promptly to family crisis situations, documents critical incidents and is available 24/7 to clients.
• Completes an initial and ongoing family assessment related to safety, stability and well‑being, including mental health, substance abuse, education, emotional, social and physical well‑being, parenting and parent‑child relationships.
• Assesses each child’s safety with every family contact and reports all suspected child abuse/neglect.
• Monitors family progress and service provision through the use of assessment tools, and updates family case plans based on those assessments.
• Appears and testifies in court as required.
• Provides 24‑hour, 7‑days a week support to clients and remains available during crisis situations.
