Phoenix Home Care and Hospice provides compassionate nursing care to clients and their families dealing with a terminal diagnosis. The Case Manager works with the interdisciplinary team to develop and implement a patient‑ and family‑centered plan of care, facilitates goals‑of‑care discussions, and guides healthcare decisions to enhance quality of life.

We are Medicare Certified and State Licensed.

• Provide education to patients/family members regarding the process, plan of care, and offer guidance with healthcare decisions.
• Consult with the attending physician and Hospice Medical Director regarding changes in patient condition that require order changes and/or changes to the plan of care.
• Coordinate hospice care by working with other team members and with the patients/family members to ensure needs are met.
• Conduct initial as well as ongoing comprehensive assessments.
• Perform evaluations for hospice care along with informational visits.

• Kansas RN License
• Hospice experience preferred but not required.
• Valid driver's license and auto liability insurance.
