Position Summary:

This position provides support and oversight to the case management department and reports directly to the Hospital Case Management Director. This RN CM participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. Provides administrative support to the Case Management Department. These functions include assigning staff referrals, messages and patients to daily assignment lists, timekeeping duties, orientation and training of new staff and serving as the leader in charge when the manager and director are unavailable.

Job Responsibilities:

  • Provides case management services for both inpatient and observation patients as assigned.
  • Demonstrates the ability to adapt technique/treatment to age of population served, including newborn, pediatric, adolescent, adult and geriatric patients, while providing care/service with respect to cultural differences and spiritual needs.
  • Demonstrates responsibility and accountability in the coordination of case management office and staff daily assignments.
  • Ensures patient/family/other communications are forwarded to staff as requested.
  • Ensures accurate daily timekeeping, follows guidelines for all deadlines and handling exceptions, inputting base schedules, changes, call pay, and making final corrections
  • Coordinates, attends, and processes communications before and after meetings as requested
  • Courteously and professional handles department calls, maintains staffing minutes, shared drive and department web page.
  • Provides faxing, filing, reviewing, and processing of all department invoices.
  • Approves requests as appropriate for medications, transportation, equipment and other items needed to dismiss patients while ensuring compliance with ethics and compliance regulations.
  • Assists with audits and compliance with case management regulatory guidelines per CMS.
  • Manages VPro utilization for case management vendors
  • Assists in setting up new employees with office space, computer and phone access
  • Assists with patient and employee rounding and documents appropriately.
  • Evaluates progression of care using evidence-based tools and approved criteria (Inter Qual) throughout the episode of care; escalates progression and transition of care issues through the established chain of command
  • Facilitates patient throughput with an ongoing focus on an effective care transition, quality and efficiency
  • Participates in performance improvement activities including, but not limited to, identifying, documenting and intervening when avoidable days occur
