JOB TITLE:Support Worker (Care at Home)
JOB TYPE:37.5 hours per week
Working Pattern: 4 days on, 4 days off, on a rolling rota (12-hour shifts).

We are seeking a dedicated and compassionate Support Worker to join our team. In this role, you will provide high-quality support to individuals, helping them live safely, independently, and with dignity.
KEY REPSONSIBILITIESAs a care at home support worker, you will:
- Provide person-centred support to individuals in their daily activities
- Promote independence, choice, and wellbeing
- Maintain accurate and professional care records
- Communicate effectively with service users, families, and external agencies
- Work flexibly as part of a committed team
- Work with support plans, risk assessments, and policies
- Assist with medication administration
