Prosper at Wickliffe is seeking a motivated individual to serve as an LPN in our assisted living community! The LPN works with the Interdisciplinary team, resident, and family to assess and determine needs within an Individual Service Plan (ISP) and coordinate service provision as identified in the ISP. The LPN is responsible for working with the Resident Services Director and Caregivers to provide optimum care for each individual resident.

The LPN is responsible for general supervision of staff as well as disciplinary action when needed. Also during the course of duty, the LPN may be called upon to perform various technical nursing skills.

• Assisting staff in the provision of direct care to residents affords not only the opportunity to serve as a role model but also provides the LPN the opportunity to assess caregivers' skills and a better recognition of the supports and constraints under which the staff function.
• Function as liaison between staff and administration, staff and families, staff and service providers including physicians to promote optimal care for the resident on both a formal and informal basis.
• Works with the Resident Services Director to develop and coordinate the community educational programs including, but not limited to, orientation and in-service for all personnel.
• With the Resident Services Director, directs, evaluates and supervises all resident care and initiates corrective action as necessary.
• Reviews residents admitted and implements with the interdepartmental team the Individual Service Plan for each resident.
• Identifies safety hazards and initiates corrective action as necessary.
• Completes reports and assignments accurately and adheres to established time schedules.
