The post holder will be responsible and accountable for the delivery of an accessible, appropriate, high quality and effective nursing service to all age groups in the community by working in partnership with patients, care providers and other agencies.
The post holder will ensure a person-centred approach to care delivery that is safe and effective, maintaining a quality strategy.
The post holder will function as an independent autonomous practitioner by virtue of in‑depth knowledge, expertise, proficiency and experience to lead and enable the Community Nursing team to provide a quality nursing service.
The post holder will be a clinical expert, be involved in public health activities and be a key member of the Primary Care team and other multidisciplinary groups.
In addition, the post holder will be responsible and accountable for the leadership and management of the aligned community nursing team, linking into the management structure of the Locality.
What we need from you- Registered Nurse with valid NMC Registration
- Educated to or working towards a degree level
- Postgraduate leadership experience demonstrating the breadth of knowledge to lead safely, effectively and efficiently
- Comprehensive knowledge of clinical guidelines and standards within the speciality/area of practice
- Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice
- Management qualification or equivalent gained through experience and continuous professional development
- Evidence of continuous professional development in the area of speciality, including study days, courses or postgraduate qualifications
- Excellent team‑working and leadership skills with the ability to motivate others and work using own initiative
- Excellent listening, communication and interpersonal skills
- Effective time‑management skills
- Working knowledge of basic information technology
A driving licence is required; this must be a full UK/EU/EEA licence.

Fixed Term (1 Year) - Full time, 37 Hours
