Widnes Primary Care Network is launching an innovative newneurodevelopment (ADHD) service and are looking for a passionate, skilled, and motivated Neurodevelopment Practitioner to join our growing team.

This is an exciting opportunity to help shape a new service that supports the adult patients of Widnes GP practices who are seekingassessment for ADHD. Throughhigh-quality neurodevelopmental assessment, intervention, and ongoing support.

You will work within a multidisciplinary environment that values collaboration,evidence-based practice, and person-centred care.

This is a 12-month fixed term contract with the potential tobecoming a permanent post, salary range is £37,000 - £47,000 depending on experience.

Main duties of the job

As part of a primary care based neurodevelopmentalconditions service, the postholder will:

be the first point of contact in primary careand offer advice to adults who are managing some challenging symptoms or whomay be seeking an assessment for ADHD

Undertake needs-led stratification assessments to identify needs, support patients to access appropriate advice, guidance and education relating to ADHD and outline possible intervention and supportoptions

Under appropriate supervision, undertake ADHDannual reviews including physical health checks and discussion of ongoingmedication issues or concerns with the patient (training will be provided)

Complete clinical and appointment-related administration and record keeping

You will be employed by Widnes Highfield Health GP Federation on behalf of Widnes Primary Care Network our head office is locatedon the site of a former maternity hospital in Widnes, Cheshire. The building has since been renovated tocreate comfortable waiting areas and clinic rooms filled with state-of-the-art facilities and equipment. The settinggives us access to the latest technology, enabling accurate and timely assessment and sharing of results for our patients, forming a core part of the healthcare on offer in Halton.

The patient list size of Widnes PCN is 68,000 over 8practices.

Job responsibilities

To receive, screen, triage, stratify and streampatient referrals for adult ADHD assessments, or on-going treatment

To conduct face-to-face and occasionally remoteappointments.

To undertake comprehensive needs-ledstratification assessment in line with relevant guidance, best practice and service model, identifying needs from possible ADHD and other conditions orfactors.

To offer a range of evidence-based education,advice and guidance relating to ADHD with patients to help reduce and managesymptoms.

To manage an identified caseload of patients, in conjunction with other professionals, agencies and carers as may be required tosupport the take up of appropriate education, advice and guidance.

Under appropriate supervision, undertake ADHDannual reviews, to include a physical examination and discussion on symptom management, side effects, supporting non-pharmacological strategies and liaison/ referral to other agencies as appropriate. This may include liaison with the GP and close liaison with other primary care/ social care colleaguesfor any additional issues.

To ensure a holistic and empathetic approach to appointments and treatment is undertaken, including consideration of physical and mental health needs and signposting to other resources or referring toother services or agencies as required.

To complete clear, accurate and timely records,summarising the results of all appointments and the advice and guidanceprovided. Make recommendations for ongoing support and clinical or otherreferral when necessary.

To develop an understanding of a patientsjourney from the point of referral for an assessment of adult ADHD, including the various treatment options and their management under the specialist primary care service or shared care with the patients practice.

To support effective collaborative working across general practice, the primary care service, and secondary care provisionthough effective communication with other professionals.

To engage with and use web-based informationspecific to this patient group, maintaining up to date and accurate resourceswhich can be made available to both colleagues and patients.

To stay up-to-date and engaged with the latestresearch and developments in ADHD assessment and treatment and mental health.
