An exciting opportunity has arisen for a highly motivated and creative individual to join the Learning Disabilities Service for Mersey Care NHS Foundation Trust. You will be based with the Halton Community Learning Disability Team and cover the population of Halton (Widnes and Runcorn).

Working within this established multi disciplinary team, the post holder will provide specialist Physiotherapy assessment and intervention as part of a team approach to supporting service users with learning disabilities and complex needs.

As part of an Highly Specialist Physiotherapist's role, you will be in an ideal position to support and contribute to Physiotherapy/Learning Disabilities Strategic and Team Development

Being part of Mersey Care NHS Foundation Trust, you will be given opportunity to network across different learning disabilities services and liaise with other PT's and AHP's within the wider Trust linking into the AHP workforce.

You should have a keen interest in working with this client group and be able to demonstrate the ability to work as part of a Multidisciplinary Team, demonstrate leadership skills.

Operate within the principles of continuous quality improvement. Will monitor and critically analyse relevant data to help support and implement change.

Participate as an active member of the MDT in developing and delivering high quality and innovative services, promote effective teamwork, delegating activities appropriately, to the benefit of the patient and service delivery. Expectations will be achieved through hands‑on clinical practice, education and training and research.

Be expected to effectively manage a specialist and complex caseload of patient's co‑morbidities as an autonomous practitioner, using evidence based/ patient centred principles to assess, plan, implement and evaluate interventions of the patient group. The post holder will be responsible for the planning, development and evaluation of Physiotherapy Services within a designated area/team.

Take a holistic approach in the assessment and treatment of service users / patients, being responsible for the provision of appropriate, high‑quality care to a defined caseload of a complex nature, using the appropriate model of care and evidenced based practice.

Postholders will work in partnership with other professionals and agencies to enable patients/clients to be maintained in an appropriate environment. To work within Trust clinical guidelines, HCPC and Chartered society of Physiotherapy (CSP) guidelines and to have a working knowledge of national and local standards and monitor own practice against these.
